Do my employees need appointments?
Appointments are preferred, but not required. Our clinic staff will assist you and your employees with making appointments and also explain “walk-in” access when needed.

How do I start using Occupational Medicine and Wellness?
That’s easy. Call 319-575-5610 to discuss your company’s needs with our representative. Our staff will explain our services, clinics and fees. Once you have an account set up, you can begin using our services. You may also click on the link below to access the Client Profile Form, fill it out and fax to number listed on form. An occupational health representative will call you within 48 hours to discuss your needs. Please note: You may access our services prior to speaking to a representative or filling out the profile form if needed.  The account profile form simply assist our staff in performing the correct procedures and billing your company accurately.
